hdr light bleed | Too many vertex format changes.....

I have a beta running on my server that i have added added hdr lighting to
but i am getting some errors that i cant seem to fix. Please help

map name is - kb_small_01_hdr
server name is - aworkingpig.co.uk - testing area
IP is - 217.163.30.215:27015

1. grenades / rpg / smg grenades dont animate when they explode
2. I get light bleed on certain walls (see pics)
3. and i get this wierd error message in the console. (when looking at light bleed
