a system like this could be made as simple or as complicated as one would like
imho, the best way would be to add the images in the mysql database, much like imagehosters do. Its also possible to keep using external image hosters offcourse.
This way you can save a lot of metadata very easily for each image. For instance, you could create keywords, small descriptions, etc for each image. With the use of keywords, each image can offcourse fit in multiple categories: a scary house can fit in "buildings" as wel as in "horror".
as for abuse: i would say:
-only allow registered users to upload pictures
-make a group on the forums that (much like news posters can) can moderate these pictures. They would have to "activate" a picture before it can be used. This way its also possible to say to user "xxxPorn" that he'd better be carefull not to upload pictures like "blondtakinghugedildo.jpg" again, or he'll be banned
seriously, i think this could work very nicely. The question is offcourse, will Blink take the extra traffic?
