A L4D item problem that has got me stumped

I currently have 3 maps in my campaign.

The saferoom at the end of map 1 has a info_changelevel, a landmark with it's name in the info_changelevel and several items of health, weapons and ammo. When the survivors enter the saferoom, map 2 loads and the items from the previous map appear as normal along with the survivors.

The saferoom at the end of map 2 has a info_changelevel, a landmark with it's name in the info_changelevel and several items of health, weapons and ammo. Entity wise it's exactly the same as the saferoom at the end of map 2, (except for a different landmark name. When the survivors enter the saferoom, map 3 loads, the survivors appear as normal but all the items that would normally be transfered from map to map are completly gone.

I can't think of any possible explanation for this. Has this occured to anyone else or does anyone have any suggestions for solutions?

Edit: And now my map crashes to desktop at random now that I've put it into a VPK. Things are just not going my way :(
