You may have changed some of the vert weights on the arm then? Did you add a new bone in or merge any of the verts from your axe to the verts from the arm?

I merged the axe to the arm mesh because it was combined when I opened the original smd example.

Can you run me through the exact process please. I know this was a while ago but I still haven't managed to make it work properly. It's now a bigger issue because somehow part of the model is getting in front of the flashlight (although not visibly) casting a massive shadow and making the flashlight pretty much useless.

The arm and the crowbar all seem to be one mesh in the example one.

yeah looks like you selected the whole mesh when you rigged it to the hand bone. Maybe select the verts of the axe in component mode and then apply the weights?
